Trump administration rule bars Medicaid payment for pediatric gender-affirming meds, could set broader coverage precedent

Aug 24, 2026·read at STAT ↗RegulatoryNegative

The Centers for Medicare & Medicaid Services released a final rule that prohibits the use of federal Medicaid and CHIP funds for pediatric gender-affirming medications and related surgeries. The rule is slated to take effect on Oct. 13.

The administration says the policy is part of its broader effort to limit gender-affirming care for minors. Under the rule, states must stop reimbursing these treatments through Medicaid programs.

Medicaid policy experts warn the legal rationale could be applied to other drugs, potentially allowing future denial of coverage for medications that are otherwise covered under Medicaid’s usual requirement to pay for most FDA-approved prescriptions.
