Trump administration proposes 340B rebate pilot for 2027, hospitals push back

The Health Resources and Services Administration announced a new pilot that would shift some 340B drug discounts to rebate payments starting in 2027.

The proposal follows a previous attempt earlier this year that was abandoned after a court ruling. Under the plan, rebates would apply to drugs with a maximum fair price set by the Inflation Reduction Act.

Hospital groups, led by the American Hospital Association, said the change would add financial strain to already stretched providers and indicated they are exploring ways to block the pilot.

The list of drugs affected includes 25 items such as Ozempic and Eliquis, though none are the highest spend drivers in the program.
