J&J's IBD combo therapy misses statistical goal but will be tested in a focused patient subgroup

Johnson & Johnson reported that its experimental combination of Tremfya and Simponi, called JNJ-4804, did not achieve a statistically significant improvement in a Phase 2b study of inflammatory bowel disease patients. The DUET trial, which evaluated the drug in both ulcerative colitis and Crohn's disease, showed no clear benefit over existing treatments.

Despite the negative readout, the company said it will move the program into later‑stage testing, concentrating on a narrower subgroup of patients who may respond better. J&J believes the biology of the disease and the prior 2022 trial, which had shown promising remission rates, justify continued investment.

The decision reflects a broader industry trend of pursuing combination approaches for IBD, even after mixed trial outcomes. Investors and clinicians will watch the next phase closely to see if the focused strategy can deliver the hoped‑for efficacy.
