New fee-for-service maternity billing to replace bundled global payment in 2027

The American College of Obstetricians & Gynecologists (ACOG) announced that starting Jan. 1, 2027 the CPT codes used for maternity care will shift from a bundled "global obstetric payment" to a fee‑for‑service model.

Under the current bundled system, prenatal, delivery and postpartum services are combined into a single charge, leaving families with confusing bills that do not show the true cost of each component of care.

ACOG says the fee‑for‑service approach will give patients clearer cost‑sharing information, improve access to personalized care, and help track maternal health outcomes more accurately.
