About the Role: At Plume, we envision a member experience that provides thoughtful, expert, timely, and gender-affirming care that celebrates and enhances the quality of life for every trans person. As a Care Coordinator, you are the patient's first point of contact. You ensure excellent patient care and satisfaction within a fully remote, telehealth environment, reporting directly to our Care Coordinator Manager.
